External Site Code Application
Institutions seeking to deliver courses or programs at a new external site must submit the Off-Campus Instructional Site Request Form to obtain administrative approval from the USG Office of Academic Affairs at least 60 days prior to the implementation of the offerings at the site. This request form requires Smartsheet login; you may need to click the form link again after logging in. ALL form fields are required unless otherwise noted. Please complete a separate form for each requested off-campus site. (For additional requirements regarding external sites, see Academic and Student Affairs Handbook 2.3.7.1)
All approved external sites will receive an external site code and are listed in the USG External Sites Report.
